- Download (or retype) and complete the
UCSF Personal Statement
questions that must accompany your Supervised Practice Program Application.
- Please print out the
Application Instructions page for reference
during the application process. The Application Instructions provide detailed information about required
contents that must be included in your UCSF Application Packet. Please follow the application instructions carefully. Please read, at the very least, the
Program Overview page and the
Admission Requirements page
before beginning the application process. However, we strongly suggest that you read all of the information found on our
UCSF Dietetic Internship website.
